Got an issue with my bridge plate. The rear 6 holes are worn down about half or less than the original thickness, and pieces of the remaining plate continue to chip off to the point of the holes being on the top ply instead of the plate. The remaining plate is fine, and I don't think it needs to be removed. What are my options?

It's normal for them to be chipped. They drilled the hole without backing them up, blowing out the back. I make a bridge plate patch from 1/16" maple and epoxy it on top of the old one, then drill & taper ream to clean up the holes. Use a few layers of aluminum foil pressed around the bridge plate to get the shape, measure it and make a cardboard template, trim the template to the shape of the existing plate (by feel), and transfer the shape from the template to the maple.
